The Complete Guide to Therapy Insurance Coverage in Ontario

Updated August 2026

Therapy is not covered by OHIP, but most extended health insurance plans in Ontario do cover psychotherapy with a Registered Social Worker (RSW). Annual coverage typically ranges from $500 to $2,000. Insurers that commonly include it are Manulife, Sun Life, GreenShield, Blue Cross, Canada Life, TELUS Health, and Desjardins. To confirm your benefits, ask your insurer: “Do you cover MSW RSW psychotherapy?” Virtual therapy sessions are covered on the same terms as in-person under post-COVID standards.

You probably have a few questions before you book. For most people with employer benefits, that coverage is real. You might have thousands of dollars in coverage waiting. Money you’ve been paying into without realizing it was there.

What’s in this guide.

Is therapy covered by insurance in Ontario?

Yes. Most extended health plans from employers include mental health coverage, and therapy and counselling are both typically covered under that category. Annual limits usually run between $500 and $2,000 per person per year.

The real variable isn’t whether therapy or counselling is covered. It’s who provides it. Insurance plans list the specific credentials they’ll reimburse. Most Ontario extended health plans recognize Registered Social Workers (RSWs), which is the credential held by Inner Room’s entire team. Some plans also cover psychotherapists more broadly, and some include psychologists, though psychologist sessions generally cost more out-of-pocket.

Some plans require a referral from a GP, many don’t. Virtual therapy in Ontario is covered by most plans the same way as in-person sessions. That’s a relatively recent shift, but it’s now standard.

The only way to know your actual coverage is to reach out to your benefits provider directly. Check your benefits card or your HR portal, find the phone number, and ask: “Does my plan cover Registered Social Worker (RSW) sessions, and if so, what’s my annual limit?” You’ll have your answer in five minutes.

What credentials does your therapist need for therapy to be covered by insurance?

Insurance plans typically list covered professions. You’ll usually see something like: RSW (Registered Social Worker), RP (Registered Psychotherapist), or Psychologist.

Inner Room’s therapists are all MSW RSWs — Registered Social Workers with master’s-level training, regulated by OCSWSSW. This matters for insurance. Most extended health plans in Ontario require an MSW RSW specifically. Some RSWs hold a BSW (Bachelor of Social Work) instead — and most plans won’t reimburse BSW RSW sessions. When you call your insurer, ask: ‘Do you cover MSW RSW psychotherapy?’ That’s the specific designation to confirm. If you check your benefits and see “RSW” on the list of covered professions, your Inner Room sessions will qualify.

RSW coverage is widespread across Ontario. Most people who take five minutes to check find it’s on their plan. Always verify first, but the odds are in your favor.

A quick note on accuracy: Psychologists are a distinct regulated profession, separate from RSWs. They’re trained differently, regulated differently, and they cost differently. We mention this because understanding the real differences matters more than treating all designations the same.

Does OHIP cover therapy in Ontario?

Not directly. OHIP doesn’t cover private psychotherapy.

OHIP covers psychiatry, which means medical doctors who specialize in mental health, usually medication-focused. That’s a different thing from therapy with a social worker or psychotherapist.

Some community mental health programs are OHIP-funded and free, but they typically come with waitlists of 3 to 6 months.

Private therapy means no waitlist, usually no referral required, and you can start this week if you want to.

What if I don’t have extended health benefits?

A lot of people don’t. Self-employed people, part-time workers, contract workers: tons of Ontarians are outside the standard employer-benefits system.

You still have options. If you’re a student or under 25, check if you qualify for coverage under OHIP+. Some workplaces offer EFAP (Employee and Family Assistance Programs) that include a few free sessions, usually 3 to 8, before you’d pay out-of-pocket. Community mental health organizations sometimes offer sliding-scale fees based on income.

Inner Room offers student rates, and we offer a free 15-minute consultation so you can ask questions and figure out fit before you commit to anything. It’s a real conversation, not a sales call.

What about Ontario Works or ODSP?

OW and ODSP don’t directly pay for private psychotherapy sessions. There’s no mechanism in either program that routes payment to a private therapist.

That said, recipients have real access to mental health support at no cost through publicly funded routes.

The Ontario Structured Psychotherapy (OSP) program, run through Ontario Health at ontariohealth.ca, offers free CBT-based therapy for anxiety and depression through community-based providers. It’s available to anyone in Ontario regardless of income, including people on OW or ODSP. Regional availability and waitlists vary, and many sessions are now delivered virtually. Check ontariohealth.ca to see what’s currently available in your area.

Community health centres and family health teams offer free counselling through regulated social workers. Sessions are typically short-term, usually 6–8, and often come with a wait of several months. They’re real programs with real clinical support.

On the OW or ODSP side specifically: discretionary benefits are a category worth raising with a caseworker directly. In some circumstances, provisions may apply to mental health services. No guarantees, but it’s a straightforward question to ask at your next appointment.

If you’re on OW or ODSP and want access to ongoing private therapy, some practices offer sliding-scale fees based on income. It’s always worth asking.

How do I submit a therapy claim to my insurance?

After each session, Inner Room provides a receipt with everything your insurer needs: the session date, duration, the type of service, and your therapist’s name and registration number as an RSW.

You submit that receipt to your insurer through their app or online portal. Most plans have moved online by now. Reimbursement typically takes 5 to 10 business days.

Some plans have a deductible, some reimburse 80% of the fee, and some reimburse 100%. Check your plan details to know what your out-of-pocket might be on each visit.

Frequently asked questions about therapy and insurance in Ontario

Does Sun Life cover therapy in Ontario?

Yes, most Sun Life plans include coverage for MSW RSW psychotherapy. The specifics depend on your individual plan: the annual limit, whether reimbursement is 80% or 100%, whether a referral is needed. A quick call to Sun Life confirms all of it in minutes. When you call, ask specifically: “Does my plan cover MSW RSW psychotherapy, and what is my annual limit?”

Does Manulife cover therapy?

Most Manulife plans cover MSW RSW sessions. Coverage limits vary by plan, and some plans reimburse 80% while others cover 100%. The fastest way to find out where you stand is to call Manulife directly or check with your HR department. Ask for your mental health benefit limit and whether MSW RSW is an accepted credential.

Does Blue Cross cover therapy in Ontario?

Yes, typically. Blue Cross plans usually include coverage for RSW and other mental health practitioners. The key is verifying that the credential type your plan accepts matches your therapist’s designation.

Is online therapy covered the same as in-person?

For most Ontario extended health plans, yes. Virtual therapy sessions in Ontario are treated the same as in-person since coverage policies updated post-COVID. Your receipt and reimbursement process are identical.

Can I get therapy for free in Ontario?

n some cases, yes. If your employer offers an EFAP (Employee and Family Assistance Program), you likely have access to 3 to 8 free sessions before any cost kicks in. Check with your HR department first. The Ontario Structured Psychotherapy program also offers free CBT-based therapy for anxiety and depression through Ontario Health, though waitlists vary by region. Students at Ontario universities often have free on-campus counselling, and community mental health organizations offer free or sliding-scale services. If cost is a real barrier, ask about it directly during your free consultation. There are options.

What is the 3-month rule for mental health coverage?

Some group insurance plans include a 90-day waiting period before mental health benefits become active. This typically applies if you’re a new employee or just joined a new plan. During that window, your extended health benefits may not yet cover therapy sessions. Call your insurer and ask specifically when your mental health coverage starts. If you’re between jobs or waiting for a plan to kick in, community mental health programs or sliding-scale practices can bridge the gap.

Do I need a referral from a doctor to see a therapist?

Usually no. Most private therapy practices, including Inner Room, don’t require a GP referral to book. You can reach out and set up a session directly. Some extended health plans do ask for a referral before they’ll reimburse, so check your plan details. That’s a different question from needing one to actually see someone.

Does my therapy receipt need anything specific?

Yes. Your receipt should show: the session date, your therapist’s name, their registration number (RSW), the session duration, and the fee paid. Inner Room provides all of this automatically. When you submit to your insurer, that’s all they need.

The real question

The question about insurance coverage is practical, and it’s worth asking. But it’s not really the hard part.

The hard part is deciding that you’re worth this. That you’re allowed to take an hour and think about yourself instead of everything else. That you deserve to understand what’s going on in your head and your life. Starting therapy takes courage, whether it’s your first time or you’re coming back after a break.

If you want to know what to expect once you do book, we’ve written a guide to what happens in your first therapy session. And if you’re wondering what sessions actually cost, we’ve broken down therapy costs in Ontario.
